Lifetime Vitality employs thoroughly vetted, outcome-focused, licensed therapists, care managers, and care coordinators. Our therapists provide each patient with clinically sound and evidenced-based rehabilitation while monitoring outcomes to prove the efficacy of our services.
We believe every patient deserves to pursue lifelong vitality, and our care providers work collaboratively with them and their families to customize goals based on what is most meaningful to the patient and their families. You can expect our therapy teams to work closely with each other, as an inter-professional team, as well as with the medical community to ensure every patient is treated with dignity and respect. Our care managers and care coordinators also are versed in healthcare gaps and are poised to assist with care integration between providers and sites of care, including in a patient’s own home.
Additionally, our licensed therapists continuously pursue clinical excellence and practice at the highest level of their unique skills. Our care managers and coordinators are thoroughly trained and dedicated to serving the older population with empathy and compassion.
Behavioral Management Integration
Behavioral Health services include systemic assessment and monitoring of mental health, substance use, stress, diet, care plan revision for patients who need additional support and continuity of care with an appointed care team member.

At Lifetime Vitality, we are concerned about a patient’s overall health, wellness and functional independence. Our healthcare team is committed to connecting patients with the resources that ensure they maintain their health even when they are not actively in therapy.
Occupational Therapy
Our Occupational Therapy teams to advocate and work tirelessly to help patients regain their function for activities of daily living including eating, bathing, dressing, grooming, cooking and cleaning.
Physical Therapy
Our Physical Therapy teams advocate and work tirelessly to help patients regain their strength and balance, improve walking, increase range of motion and decrease pain.
Remote Physiologic Monitoring
Lifetime Vitality partners with providers to remotely monitor vital signs using Bluetooth-enabled devices. Our technology not only captures key vital-sign readings into a database but also allows for audio and visual interaction with the patients, including telehealth visits when needed. Our team remotely monitors the patient’s physiologic data, triaging and if necessary, alerting the patient’s physician.
