Alert Triggered Low Blood Sugar

  • Recent DC home from Rehab
  • Medications given as prescribed on discharge orders
  • Within 4 days starting on RPM
  • 40mg/dl overnight
  • Severe hypoglycemic symptoms

Care Team Interventions

  • Advised to hold on evening diabetic meds
  • Blood sugar jumped to 400 mg/dl at dinner triggering alert
  • Physician contacting patient, further adjustment of Diabetic medication dosage

Clinical Outcome

  • Stable blood sugar readings for 60 days
  •  Safe range 70 – 200mg/dl
  • Avoided re-hospitalization
  • Safely titrated medications remotely
  • Decreased stress on patient and wife by providing virtual care (hardship for patient to exit the home)

  • Patient chose not to take her daily Aspirin thinking it was for her HTN
  • Patient has history of DVT

Care Team Interventions

  • Care team contacted patient did medication and medical history review
  • Identified patient is not on HTN medication, and she withheld Aspirin for 2 days
  • Educated patient on indication for Aspirin use with her history of DVT
  • Care team contacted patient PCP to advise for timely follow up

Clinical Outcome

  • PCP office alerted of patient’s low blood pressure readings
  • Initiated PCP reach out to patient to address patients’ medication and medical needs
  • Identified medication change that could have resulted in major medical episode
